Make copies of these worksheets before continuing, so that you can complete each of the forms and keep them in a safe place for future reference in the permanent site record.
Use the Five-Slot Chassis Installation Checklist to confirm that you have all the information needed to install and configure the chassis, and that all the components are installed and operating correctly. Each item in the Five-Slot Chassis Installation Checklist (Table A-1) should be verified by the person performing the installation and dated for the permanent site record.

Use the Site Log (Table A-4) to keep a maintenance history of the chassis. A detailed description of actions performed and symptoms observed should be completed by the person performing the maintenance and kept as part of the permanent site record.

The installation and configuration of the chassis can be separated into specific steps. If each step described in the Site Log (Table A-4) is completed, chassis installation should be successful. If the chassis fails to function properly in the first attempt, you can use the Site Log to systematically check that each step was performed correctly.
Copy the Site Log and initial each procedure as it is completed. Each task should be verified by the person performing the installation and dated for the permanent site records.
